WHAT TO BRING TO YOUR APPOINTMENT |

You
will be asked to complete two information forms
at your first exam. The first form is for general
information (address, insurance, etc.). The other
form is a Medical History Form. Please bring your
insurance cards and a list of the medications you
are currently taking. Please also bring your glasses,
contact lenses and your current eyeglass prescription,
if you have it. |
